Copy your insurance card(s) and send as directed above. If you are an established patient and your insurance has changed since your last visit, copy your new insurance cards and send by fax or mail.
Be prepared to pay your co-pay or deductible.
We accept all major credit cards and debit cards.
Non-insured patients will be given a discount if payment is made at time of service.
Schedule an appointment to be seen by a physician.
Walk-ins for established patients will be seen by the nurse practitioner if the physician is not available.
